Oasis Hospice has provided the highest quality care to patients and families living with advanced illness, while honoring human dignity and personal choice.
Oasis strives to treat the whole person — fulfilling physical, psychosocial and spiritual needs — and views our patients and their loved ones as a unit.
Patient and Family play a crucial role in the hospice team. We will work closely with you and your loved ones to identify and prioritize the goals that matter most to all of you in your care plan.
The nursing staff consists of skilled and experienced Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs), they deliver hands-on care, assess patient needs, and manage pain and symptoms. The Nursing Team offers support and education to family members on how to care for their loved ones.
Hospice Physicians / Medical Directors lead the Medical Team and can include the patient’s primary care provider; our doctors develop a care plan to manage pain and control symptoms related to the patient’s terminal illness or disease.
Hospice Aides, including Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) assist with personal care, daily activities, and provide companionship and emotional support to patients and families.
Social Workers help patients and families address communication, conflict resolution, and other stressful situations, while coordinating community resources and assisting with non-medical matters such as finances and end-of-life concerns.
Chaplains and Spiritual Care Counselors provide emotional and spiritual support, helping patients and families navigate end-of-life questions, decisions, and challenges while fostering healing and strengthening family relationships.
Our Bereavement Coordinator provides compassionate support to patients and loved ones during the end-of-life process and offers grief counseling and ongoing support for up to 13 months after a loss. They are dedicated to helping families navigate grief and find healing during this difficult time.
Volunteers bring comfort, companionship, and support to patients and families. Whether through conversations, creative projects, or providing respite, they help create an "oasis of peace" and make a meaningful difference in the lives of those we serve.
